In the 2022 Final Rule, CMS showed its continued support for telehealth and remote patient monitoring under CPT code 99453, 99454, 99457, 99458.

The key difference between RPM and CCM is that RPM allows for providers to be reimbursed for remote treatment and management of patients using devices that collect essential physiologic data. These RPM CPT codes include reimbursement for both collection and interpretation of physiologic data from devices (e.g., scales, pulse oximeters, blood pressure cuffs, etc.). CMS also requires data from devices to be collected and transmitted electronically to bill for these codes, i.e., data cannot be self-reported by the patient.

View list of patients with total measurements and pending measurements in a 30 day period. Using this info a medical assistant can make sure that 16 measurements on unique days in a 30 day period is achieved for the patient, thereby making sure that CPT 99454 reimbursement is eligible.

Monitoring time: Track monitoring time automatically or manually insert time entries. Time can be tracked on Web Portal or Mobile/tablet apps. 

Interactive time: If Patient is using our patient mobile app, then a Clinician can initiate Video call, else the Clinician can place a call outside our system and insert the interactive time entry on the web portal.

How does it work?

RPM steps as follows:

Step 1

Patient searches for your Clinic app (white labeled DocToDoor app) on AppStore or PlayStore and signs up as a new patient.

Step 2

Enroll patient: Identify and Enroll Patient into Program (Require Patient Consent). Patient receives text and email to download and access the App.

Step 3

Assign Plan: Template CarePlan assigned to Patient by the clinical staff using DocToDoor Portal. Setup Programmable Alerts (i.e. BP value within certain range) & educate Patient how to use Device(s).

Step 4

Automated Goals, Exercises, Reporting Measures and Symptom trackers assigned as part of CarePlan.

Step 5 (Optional)

Monitor and Manage patients with the mobility of Clinician App and securely Chat (optional) with them building trust and assurance.
